Request for a work excuse

 Some employers require documentation that you had attended a funeral to receive personal time off.  Please fill out the below form and within 24 hours of applying for the work excuse we will email it to you for your employer.  WE DO NOT ISSUE work excuses if no funeral or memorial services had taken place.  WE ONLY verify you had attended a funeral service for time off from work.
